In order to process your order we need you to authorize us to charge your credit card by fax or e-mail. It must have your billing address, credit card number, expiration date, and signature. If you prefer to send us a check, our mailing address is South Bay Furniture, Attn: Internet Order, 14928 Camden Ave. San Jose, Ca. 95124. Your credit card will be charged as Country Wood Furniture, Inc.
(Make any checks Payable to Country Wood Furniture).
As for ordering you can click the form below and print it out. Please fill out the Personal info, Item Description, Sku #, Price, and Quantity. Then sign and date it. If you need more space please print out another form and add them there. All you have to do now is fax it to the number on the upper left hand corner of the pay by fax sheet…
You’ll get a copy of your receipt by fax or e-mail as soon as we get your order. If you would like a copy mailed to you just let us know. We will put the estimated delivery time, tax (If you live in California) and shipping cost (if shipped), on your receipt. If your in the San Jose bay area, and would like to pick your items up, we will call you when you items arrive. Otherwise we will let you know when you items are ready to ship. (Shipping company will call you to arrange the actual delivery time).
